We’re so glad you’re interested in becoming a registered parishioner of St. Damien’s! To officially join our parish community, please fill out the registration form below. This form will ask for some basic details about you and your family, and your responses will be shared with the Archdiocese of Oklahoma City for record-keeping purposes.

The form is fairly straightforward, but we recommend setting aside about 5 minutes to complete it. Once submitted, you’ll be added to our parish records.
